The recurring cost of ADHD medication in England is the per-item charge. The NHS publishes two prepayment options and states the break-even point for each in its own words.
This is the one genuinely predictable cost on the whole pathway, which makes it worth getting right rather than guessing at.
The National Health Service states that the current prescription charge is £9.90 per item, on a page checked on 15 September 2026 whose own review stamp reads last reviewed 23 November 2023, next review due 27 November 2026.
The per-item part catches people out. Prescription charges are for each item not each prescription, and the NHS gives its own worked example: if your prescription has 3 medicines on it you will have to pay the prescription charge 3 times.
The break-even lines above are the NHS's own, not an estimate made here. The same NHS page also lists a separate hormone replacement therapy certificate at £19.80, which is not relevant to ADHD medicine but sits alongside the others and is easy to misread as a general option.

During a titration period the medicine or the dose is being adjusted, which can mean more separate items collected in a short window than a settled month would suggest. If you are working out whether a three-month certificate pays for itself, count the items you expect in that window rather than the items in a typical month later on.
If you are exempt from prescription charges, none of this applies. Exemption categories are set nationally and published by the NHS on the same charges page.
The counter charge is distinct from who pays for the prescribing arrangement. NHS South West London ICB states that the cost of any ADHD prescriptions, from both NHS and private organisations, will be covered by Right to Choose, that this includes first prescriptions to help stabilise medication, and that it also covers ongoing prescriptions if a GP does not agree to take over prescribing through a shared care protocol.
